Media Release

By signing this form, I give permission for my child's image to be used in photographs, videos, website and other publications by Bahamas Faith Ministries International and its ministries. The use of the image(s) by the Children's Church will be for educational, inspirational and promotional purposes only.

Liability Waiver

I approve of my child's participation in Bahamas Faith Ministries Children's Church and its indoor, outdoor and virtual classroom activities. Occasionally classes are held outdoors on BFM's property. A separate waiver and cover will be offered for participation in any off-property activities or events. In the event of an emergency we will contact you immediately. Should it be necessary, I authorize the provision of medical assistance to my child until emergency personnel arrives. I will not hold the chuch, its employees, members, agents, ministers or volunteers liable for any damages to myself or my child.
